Leapfrog Recognizes Harris Regional Hospital With ‘A’ Safety Grade for Preventing Patient Harm

Harris Regional Hospital, a Duke LifePoint hospital, has received an ‘A’ Hospital Safety Grade from The Leapfrog Group for fall 2025. The grade is based on a national set of evidence-based measures that evaluate how well hospitals protect patients from preventable harm, such as medication errors, infections, and complications.
The Leapfrog Hospital Safety Grade is updated twice a year and is recognized as one of the most comprehensive assessments of hospital safety performance in the United States. Grades are calculated using more than 30 publicly available indicators, including staffing patterns, hand-hygiene practices, use of electronic medical records, patient-reported concerns, and outcomes related to procedures and hospital stays. The methodology is peer-reviewed and available for public review.
The recognition reflects ongoing work across all departments to strengthen safety processes, address risks proactively, and maintain reliable systems that support high-quality care.
